Patient Forms

For new patients, we have our New Patient Agreement and Patient Records Release Forms that can be completed and submitted to med2you-help@outlook.com. These forms can also be printed and brought to your initial visit. By completing these prior to your initial visit, we can get you in and out and back to your busy routine without delay. The New Patient Agreement is required for all new patients, while the Patient Records Release Form is required to obtain previous documentation from past providers. This helps us to make sure that your medical records are up to date to provide the best care possible.
